Forms for Child Care Assistance Program

If you are using the Google Chrome browser:
At the “Please Wait” error page, you can choose to download the PDF by clicking the download button… it is a downward pointing arrow, ending at a line. You can then open the file in your installed PDF reader program.

Child Care Assistance Application. Complete this form if you are a new applicant or if your child care case has been cancelled for over 30 days.

Letter of Employment Requirements Form. Your employer will need to complete this form if you have been employed less than 30 days and/or are unable to submit 2 current, consecutive and complete check stubs.

Self-Employment Records Form. If you have been self-employed less than one year and have not yet filed income taxes, three full months of self-employment records should be completed.

If you are using the Mozilla Firefox browser, some configuring will be needed:
1. Open Firefox
2. Click the 3 stacked lines at the top right of the browser window
3. Click “Add-Ons”
4. Click “Plugins”
5. Choose “Adobe Acrobat”
6. Change to “Always Activate”
7. Close Firefox
8. Restart Firefox
9. Visit the site again
10. Click “Open With Different Viewer” in message at the top of the browser screen
11. Check the “open with” button
12. Ensure it says “Adobe Acrobat Reader…” or your installed PDF reader program.
13. Check the “Do this automatically for files like this from now on” box.
14. Click OK
15. You will be able to open the PDF form
